MEMBERSHIP CHANGE FORM
Please only fill out this form if you are making changes to an EXISTING membership or memberships. If you are adding a NEW member to your household membership, that must be done in person.
IF you are adding existing members to a household membership, this form MUST be filled out by the primary member in the household
All membership changes must be made by the 27th of the month. Any submissions sent on or after the 27th may not be processed in time for the following month.
We do not process membership changes in the middle of the month.
Members can change their membership type twice per year, and must maintain their changed membership for a minimum of 2 months before changing again.
