MEMBERSHIP CHANGE FORM

  • Please only fill out this form if you are making changes to your EXISTING membership. If you are adding a new or existing member to your family membership, that must be done in person.

  • All membership changes must be made by the 27th of the month. Any submissions sent on or after the 27th may not be processed in time for the following month.

  • We do not process membership changes in the middle of the month.

  • Members can change their membership type twice per year, and must maintain their changed membership for a minimum of 2 months before changing again.
