MEMBERSHIP CHANGE FORM

  • Please only fill out this form if you are making changes to an EXISTING membership or memberships. If you are adding a NEW member to your household membership, that must be done in person.

  • IF you are adding existing members to a household membership, this form MUST be filled out by the primary member in the household

  • All membership changes must be made by the 27th of the month. Any submissions sent on or after the 27th may not be processed in time for the following month.

  • We do not process membership changes in the middle of the month.

  • Members can change their membership type twice per year, and must maintain their changed membership for a minimum of 2 months before changing again.
