Membership Cancellation Request Form

Please Read Carefully Terms & Conditions:

• Your Pacific Edge membership can be cancelled at any time, advanced written notice is REQUIRED and cannot be done over the phone. Please fill out the form below.

• We do not give refunds on monthly billing once billing has gone through or for PIF memberships.

• Members on a family membership MUST submit a request form for each member.

• Cancellation requests made AFTER the 27th of the month may not be processed in time to prevent charges. (please refer to the second bullet point)

• Please fill in ALL fields, set the effective date you wish to cancel, and submit the form.
